Crocin Ds exhibits an analgesic action by peripherally blocking the generation of pain impulses. It produces antipyresis by inhibiting the thermoregulatory center of the hypothalamus. Its weak anti-inflammatory activity is related to the inhibition of prostaglandin synthesis in the CNS.
Crocin Ds (paracetamol) is believed to act primarily in the CNS, increasing the pain threshold by inhibiting both cyclooxygenase isoforms, the COX-1, COX-2 and COX-3 enzymes involved in prostaglandin (PG) synthesis. Unlike NSAIDs, acetaminophen does not inhibit cyclooxygenase in peripheral tissues and therefore has no peripheral anti-inflammatory effects. Although aspirin acts as an irreversible inhibitor of COX and directly blocks the active site of the enzyme, studies have found that acetaminophen indirectly blocks COX and that this blockade is ineffective in the presence of peroxides. This may explain why paracetamol is effective on the central nervous system and endothelial cells, but not on platelets and immune cells that have high levels of peroxides. Studies also report data suggesting that acetaminophen selectively blocks a variant of the COX enzyme that is different from the known COX-1 and COX-2 variants. This enzyme is now known as COX-3. Its exact mechanism of action is not yet well understood, but future research may provide more information on how it works. The antipyretic properties of acetaminophen are probably due to direct effects on heat-regulating centers in the hypothalamus, resulting in peripheral vasodilation, sweating, and thus heat dissipation.

Collateral damage
As with all acetaminophen products, adverse drug reactions are rare (>1/10,000, <1/1,000) or very rare (<1/10,000). Common injection site adverse reactions (pain and burning sensation) were reported during clinical studies. Very rare cases of hypersensitivity reactions ranging from a simple rash or urticaria to anaphylactic shock have been reported and require discontinuation of treatment. Cases of erythema, flushing, pruritus and tachycardia have been reported.
Caution
Administration of Crocin Ds in doses higher than those recommended may cause liver damage, including the risk of severe hepatotoxicity and death. Do not exceed the maximum recommended daily dose of Crocin Ds. Use caution when administering Crocin Ds to patients with the following conditions: hepatic insufficiency or active liver disease, alcoholism, chronic malnutrition, severe hypovolemia (eg, due to dehydration or blood loss), or severe renal insufficiency (creatinine clearance < 30ml/min). ). There have been rare reports of life-threatening anaphylaxis requiring emergency medical attention. Discontinue Crocin Ds IV immediately if symptoms associated with allergy or hypersensitivity occur. Contraindication

pediatric use: The safety and efficacy of Crocin Ds IV for the treatment of acute pain and fever in pediatric patients 2 years of age and older is supported by evidence from adequate and well-controlled studies of Crocin Ds IV in adults.
geriatric use: No overall differences in safety or efficacy were observed between these subjects and younger subjects, and other reported clinical experience has not identified differences in responses between elderly and younger patients.
